Re: Emergency Notification Procedure for Aircraft Incidents/Accidents - Smith Field
Enclosed is a copy of the emergency notification procedures the Fort Wayne – Allen County Airport Authority has established for aircraft accidents/incidents at and around Smith Field Airport.
As the Smith Field Supervisor responsible for the day-to-day operation of the airport, we request that you post these procedures in conspicuous locations where persons can refer to them quickly. Also, we request that you outline an employee training program for these procedures in written form and submit it to Airport Supervisor for our information.
We believe that these procedures will allow for quick emergency notification in a simple, easy to follow, format. With this program in place and with recurrent training provided to your employees, an expeditious response of emergency equipment to an aircraft accident/incident can be accomplished and time-critical information can be shared with all agencies which will respond during these types of events.
Should you need any clarification, or would like to request a change, in regard to the enclosed procedures or the type of training program we are looking for, please feel free to contact the Airport Supervisor.

Accident Response and Notification Requirements
Aircraft Accident/Incidents - Smith Field
The following shall constitute procedures to be followed when reporting and responding to aircraft accidents or incidents on or in the vicinity of Smith Field Airport.
AUTHORITY:
Jurisdictional Authority for Aircraft Accident Emergency Response and Investigation on and around Smith Field Airport is established by Federal and State law.
The following agencies have emergency or law enforcement response responsibilities depending on jurisdictional boundaries:
Fort Wayne – Allen County Airport Authority
City of Fort Wayne Fire
City of Fort Wayne Police
Washington Twp. Fire Dept.
American Medical Response
The following agencies have overall responsibility for all aircraft accident investigation under State and Federal Law.

Indiana State Police
NTSB
FAA
PROCEDURES:
The following notification procedure shall be used for aircraft accidents/incidents that occur at or in the immediate vicinity of Smith Field Airport.
1. As soon as an aircraft accident or incident occurs or information is received indicating an aircraft accident or incident has occurred, the Airport Supervisor or, in case of his absence, an authorized representative acting on his behalf will immediately:
A. Place a 911 call and advise the dispatcher of the accident/incident.
B. Place a call to the Public Safety Department and advise the type and nature of the aircraft accident/incident.
2. When possible, the following basic information should be given in both telephone calls:
A. Caller’s name and location calling from
B. Type of aircraft and number of soles on board, if known.
C. The most accurate geographical location available.
D. Request medical response if needed.
NOTE: Wait for any questions before hanging up and leave a callback number.
3. The Fort Wayne - Allen County Airport Authority Public Safety Department will:
A. Notify the Indiana State Police.
B. Notify the Airport Authority Director or his designated representative.
C. Notify the FAA Air Traffic Control Tower.
D. Dispatch Airport Authority personnel as needed in support of responding agencies based on personnel requirements and availability. At a minimum, a Public Safety Dept. Officer shall be dispatched to Smith Field to complete an Accident/Incident Report and, if requested, assist investigating authorities at the accident site.
